xen pvfb: Pointer z-axis (mouse wheel) support

xen pvfb: Pointer z-axis (mouse wheel) support

Add z-axis motion to pointer events.  Backward compatible, because
there's space for the z-axis in union xenkbd_in_event, and old
backends zero it.
